Prepare Your Evidence (Crucial)
Before you start the form, gather clear, high-quality, and official documents that prove your business's legitimacy and physical location.

Important Note: While not specifically called out as a requirement, it is very beneficial to include your industry specific business license (NMLS for example) and photos of both the inside and outside of the office. This further proves to Google that you are who you claim to be. Photos should preferably capture your business signs.
It is extremely important that addresses match perfectly on all of these documents. Small changes, such as Suite vs Ste will cause Google to maintain the suspension.
Before submitting an appeal, check to ensure that other media accounts (LinkedIn, Facebook Page, etc) do not display mismatching information, such as an old company name or a former address. These pages are cross-verified for accuracy.
